Medical Release Form - DOWNLOAD FORM

This is a one-time form that you need to fill out for your teenager before he/she can attend any trip. (Please note it has 2 pages.) It must be notarized and we have notaries available in the youth office, and on Wednesday nights. This form is also available at the 24:7 HUB.

Volunteer Packet - DOWNLOAD PACKET

Welcome to 24:7! We are excited that you want to join us as we serve the teens in our surrounding communities. Please fill out the enclosed papers and return them to the 24:7 youth office - or leave them in the box at the 24:7 HUB. Also, if you have not filled out a background check, you may pick that up at the 24:7 HUB also. This is required for all volunteers who work with the RHCC children/teens.

24:7 Permission Form - DOWNLOAD FORM

This form is required for each event your child attends... (retreats, mission trips, etc). There will also be forms at the 24:7 HUB for each current trip.

Request for Financial Assistance - DOWNLOAD FORM

If you would like to request financial assistance for a retreat, summer camp or trip, please complete this form and turn it in to the Youth Office. 

RHCC Background Check Packet - DOWNLOAD PACKET

If you have not filled this out (i.e with Children's Ministry, or with the 24:7 Youth Ministry), please print, complete, and return to the RHCC office. This must be done before you begin your time of fun and service with our teens.
